Under the oversight of our Medical Director, Dr. Parham Gharagozlou, our clinical team continuously coordinates your care, managing medications, referrals, and long-term health metrics for complex conditions like Diabetes, Hypertension, and COPD between your scheduled visits.
Q: How does Remote Physiologic Monitoring (RPM) work? A: We utilize clinical-grade digital health hardware—such as connected blood pressure cuffs, continuous pulse oximeters, and smart scales—used in your home. This equipment transmits your vital data securely to our monitoring platform. This allows our providers to detect clinical deterioration early and make precise adjustments.
Q: Are the remote monitoring devices mine to keep? A: No. All remote monitoring hardware (including pulse oximeters, scales, and blood pressure monitors) remains the exclusive property of East Bay Medical Clinics. These medical devices are loaned to you strictly for the duration of your active clinical monitoring. Upon termination of the monitoring or if you unenroll from the remote monitoring program, all hardware must be promptly returned to our clinic
Q: Is the remote monitoring program covered by insurance? A: Medicare (CMS) provides coverage for Remote Physiologic Monitoring (RPM) and Chronic Care Management (CCM) for eligible patients. For non-Medicare patients, executives, or individuals who wish to maintain clinical-grade health tracking, we offer direct-access (cash-pay) remote monitoring subscriptions.

Q: How is my care coordinated if I need to see a specialist or go to the hospital? A: As your primary medical home, we act as the central hub for your healthcare data.
